Only active staff can be assigned a form to sign

When assigning a form, now only active staff members are visible in the dropdown menu on the forms details drawer. 

Previously, all staff members were visible in the dropdown menu when assigning forms which was an issue with potentially re-assigning a form to inactive staff members.
